CYCLESSA® , the lowest estrogen dose triphasic oral contraceptive available, reduces overall hormonal exposure while maintaining contraceptive efficacy and excellent cycle control. CYCLESSA® 's low dose formulation and excellent side effect profile makes it an ideal alternative to the older, higher dose triphasic OCs.

How do I enroll?
You may enroll in the BCRx program one of four ways:

1. Online. The quickest and easiest enrollment option (see menu option bar: enroll ). 
   
2. By mail. (See menu option bar: enroll ) Print the enrollment form. Complete and sign the application. Submit the application with credit card information or a check for $19.99 (if applicable) payable to BCRx. 
   
3. By fax. (See menu option bar: enroll) Print the enrollment form. Complete and sign the application. Fax the application with credit card information to (480) 947-0474 .
   
4. By phone. Enroll by contacting a BCRx representative at 480-212-1000 or 800-311-BCRx (2279).

A BCRx ID Card will be mailed to your mailing address and a temporary card will be e-mailed to your e-mail address for your convenient and urgent use as soon as payment is processed.  All enrollment fees are non-refundable.
